Forms & Letters

Blank forms will not be accepted. Personal information must be completed.
Forms will be completed within 7 business days in most cases
Forms are only completed for accounts in good standing. Outstanding balances must be settled prior to form completion.
Many forms may require a current examination to be completed.
There is a $10 fee for forms to be completed unless they are brought to your appointment. Fees are collected prior to release of forms.
There is a $10 fee for any letter to be written by the Provider.

There is a $30 fee for the completion of FMLA or Disability Forms. This is due when forms are picked up.
These forms must be picked up in person and will not be faxed or mailed to your employer.

The practice does require a letter from a mental health provider prior to initiating HRT for anyone under age 18.
We may require a letter in certain situations on a case-by-base basis, which will be discussed at your appointment if needed.
If you have not seen a mental health provider and would like to do so before your visit, we are happy to offer the names of local providers.
